Student’s Aadhaar, one of the Parent's Aadhaar & Community certificate – Xerox
10 passport-size photographs, preferably the same photograph used for NEET/Counselling (Keep a few extra copies)
Hepatitis B Vaccination – 1st Dose (in the format prescribed in Annexure IX of the Selection committee Prospectus Pg. No.105) – view Annexure IX
Sworn Affidavit for Nativity and Community, duly executed before a Notary Public on a ₹100/- non-judicial stamp paper – view affidavit format
Things to Note:
Arrange originals in the same order as per the checklist and also bring 02 set of Xerox copies in the same order.
Keep scanned PDF of each furnished original certificates and NEET submitted photo & signature of the candidate in jpeg format in a pen drive.
