I, ……………………………………………………………… have applied for MBBS course in the State of Tamil Nadu for the academic year 2026-2027.

I assure that, I have not applied for any Educational Institution in any other State, claiming the nativity of that State for admission in MBBS/BDS course.

I agree that if it is found later that I have applied as a native of any other State, other than Tamil Nadu. I acknowledge and agree that my admission will be cancelled at any stage including after commencement of the course.

Further, during the course of study even if it is subsequently found that I have produced a false nativity certificate or given a false undertaking, it will result in expulsion of my candidature from the course and also initiation of criminal proceedings against me as well as my parents as per provision of the law.

I am also aware that my nativity certificate will be reconfirmed with relevant Revenue authorities at any time during or after admission.

I hereby declare that I have not claimed nativity in any other State, other than Tamil Nadu.

Undertaking

I, ______________________________________________ hereby solemnly give my undertaking that I have not taken up any seat either in All India Quota Government College or in the Deemed Universities in the Counseling conducted by the Medical Counseling Committee and the Additional Director General of medical education of Director General of Health Services or in ANY state counseling other that Tamil Nadu.

I am aware of the Hon’ble Supreme Court Judgment in DAR-US-SLAM case W.P.No.(C) 287 of 2017.

I am well aware of the fact that if my statement is found to be false, my allotment will be cancelled without any notice and I am liable for legal action.

Nativity Undertaking

I assure that I have not claimed nativity in any other state other than Tamil Nadu.

I am also aware that my nativity certificate will be reconfirmed with relevant revenue authorities at any time during or after admission.

At the time of verification or at a later date, if it is found that I had obtained nativity based on false undertaking and false faculty information or that I have obtained dual nativity certificate, my admission is liable to be cancelled without any notice and also I am liable for legal action.

Undertaking Regarding Community Certificate

I belong to __________________________ community and I am aware of the fact that my community certificate will be verified with relevant authorities at any time, during or after admission.

I agree that at the time of verification or at a later date, if it is found that the community certificate provided by me is false (or) incorrect and was based on false undertaking/ information, my admission is liable to be cancelled immediately without any notice and that thereafter legal action will also be initiated against me.
